In a shocking incident in a village in Koppal district, a dog suspected to have been suffering from rabies is said to have bitten 25 people on Friday [22 Dec 2023]. The incident was reported near the Primary Health Centre [PHC] of Alavandi village of Koppal district.

Currently, 8 patients are undergoing treatment at the district hospital, and the others were treated at the village PHC. The condition of 3 people, including a 4-year-old girl, is said to be
critical. The dog was caught by the local authorities, and it died of injuries later.

[V], one of the victims treated at the PHC, said that he was getting his treatment when some people came running seeking the treatment for a dog bite.

"The dog ran from one point to another, biting everyone who came in its way. In no time, the dog came at me as well and bit me. It was a horrific site, as even children were injured in the entire incident. The local authorities were quick to net the rabid dog and took it away before it could cause more damage," he said.

There are several stray dogs in the village, but this is a one-off incident, said [B], a villager.

[If a single dog, as indicated in the news report above, has bitten 25 people, there is a strong possibility that the dog suffered from the "furious form" of rabies:

"This is what is meant by 'mad-dog syndrome' ... in which the animal becomes irritable and, with the slightest provocation, may viciously and aggressively use its teeth, claws, horns, or hooves. The posture and expression is one of alertness and anxiety, with pupils dilated.
Noise may invite attack. Such animals lose caution and their fear of humans and other animals... Rabid dogs may chew the wire and frame of their cages, breaking their teeth, and will follow a hand moved in front of the cage, attempting to bite."

Among the 25 dog-bite victims, those who have been inflicted with category III exposure (single or multiple transdermal bites or scratches) must be treated additionally with rabies immunoglobulin (RIG)/monoclonal antibodies along with wound washing and immediate
vaccination
(https://www.who.int/news-room/fact-sheets/detail/rabies).

"If rabies immunoglobulin is not available on 1st visit, use can be delayed by up to 7 days from the date of the 1st vaccine dose.

"RIG should be infiltrated around the wound as much as anatomically feasible. Remaining RIG should be injected at an intramuscular site distant from that of vaccine inoculation (e.g., into the anterior thigh)." - Mod.PKB
