The moderators' apt comments in this post reiterate the need to address rabies as a regional transboundary disease One Health challenge. And urgently too.
この投稿における編集担当者の的確なコメントは、狂犬病は地域的な疾病でありつつ越境性のものであり「ワン・ヘルス」の課題として取り組む必要性を改めて示している。しかも、緊急にである。


As we approach World Rabies day [28 Sep 2022], the post reminds us of the complex challenges in the path to achieve zero dog-mediated rabies death by 2030. Having observed rabies trends in the region for more than 20 years, and given the current backdrop of pandemic COVID-19 and globalized monkeypox, this wake up call is being made to reflect over our commitment to combat 'lower hanging fruit' old timers like rabies.
世界狂犬病デー[2022年9月28日]を迎えるにあたり、2030年までに犬を介した狂犬病による死亡をゼロにするための道筋にある複雑な課題を思い起こさせる投稿である。20年以上にわたってこの地域の狂犬病の動向を観察し、COVID-19のパンデミックやサル痘の世界的な拡がりという現在の背景を踏まえ、この警鐘は、狂犬病などの古典的な「達成しやすい」取り組みを我々に振り返らせるために鳴らされたものである。

Uncontrolled stray dog populations, war-linked or not, are growing in disturbing numbers almost everywhere in the region especially in the Arabian Peninsula where avoidable human-stray dog clashes inclusive of gun violence took place as recent as last July [2022].

戦争に関連するかどうかにかかわらず、制御不能な野良犬の数は、この地域のほぼ全域で不穏な勢いで増加している。特にアラビア半島では、昨年7月にも、銃による暴力を含む、回避可能な人間と野良犬の衝突が発生した[2022]。

The outlook is grim. Recent, repeated fox sightings in various urban areas (including personal sightings) point to continued anthropogenic ecological disruptions creating more wildlife-human interface and spillover event opportunities.

見通しは厳しい。最近、様々な都市部においてキツネが繰り返し目撃されていることは(個人的な目撃例も含む)、人為的な生態系の乱れにより、野生生物と人間との接点が増え、波及する機会(訳者注;キツネから犬への感染による都市型狂犬病の発生する機会)が増えることを示している。

 

Even grimmer: unlike the jackals mentioned in the post, huge numbers of commensal hamadryas baboon (_Papio hamadryas_) are migrating and have reached cities far from their original Southern base.

さらに深刻なのは、この記事で紹介したジャッカルとは異なり、共生性のマントヒヒ(Papio hamadryas_)が大量に移動し、本来の南部の拠点から遠く離れた都市部にまで到達していることだ。

 

While the authorities are addressing the ensuing human-baboon conflicts, especially on farms, pathogen spillover can occur: the interaction of these baboons with foxes, stray dogs and cats at garbage dumps and farms where they compete for food provide the interface for such possible events.

当局が特に農場での人とヒヒの衝突に対処している間に病原体の波及が起こりうる:ヒヒとキツネ、野良犬、野良猫がゴミ捨て場や農場で餌を奪い合っていると、そのような事態が起こりうるのである。

 

Rabies positive baboons, as well as a rock hyrax (_Procavia capensis_) have been reported. Prior to this new migratory trend, these baboons traditionally occupy niches in large swathes of the Southwest of the Arabian Peninsula.

狂犬病陽性のヒヒと、ロック・ハイラックス(Procavia capensis)が報告されている(訳者注:ロック・ハイラックス:全長30㎝前後、体重2㎏前後で30頭~80頭程度の群れを作るイワダヌキ科であり、外見はモルモットに類似するアフリカ原産のイワダヌキ目イワダヌキ科に属する哺乳類)。この新しい移動傾向が見られるまでは、これらのヒヒは伝統的にアラビア半島の南西部の広大な地域で生態的地位を有している。

More permutations, albeit even more discouraging, can be written. We have shown that bats with potential spillover pathogens (e.g. rotaviruses) interact with humans through swimming pools and trees built literally in modified desert settings.

さらに落胆させられることに、さらにがっかりさせられることが報告されている。伝播の可能性のある病原体(例えばロタウイルス)を持つコウモリが、改変された砂漠の環境に文字どおり造成されたプールや樹木を通じてヒトの環境と行き来していることが我々に示されてきた。

 

Bats are endemic in the region, and despite being classical reservoirs of lyssaviruses (and other pandemic candidate pathogens) that interact with the above mentioned fauna and more, they remain poorly researched and integrated into a One Health response.

コウモリはこの地域固有種であり、リッサウイルス(およびその他のパンデミック候補の病原体)の典型的な病原体保有動物であり、上記の動物相などとも相互作用するにもかかわらず、コウモリは十分に研究されておらず、「ワン・ヘルス」の枠組みに統合されていないままである。

 


The excellent reference in the post (https://tinyurl.com/3mup3rek) re-emphasizes that molecular characterization of strains involved in animal and human outbreaks is crucial in deciphering spillover trends, used to fine-tune prevention and control interventions.

Molecular characterization of rabies strains from baboons, rock hyrax and a sandcat [see comment], will provide information on how these first-time rabies positive wild fauna, in Saudi Arabia, became spillover victims.

With huge numbers of baboons in the Southwest of the Arabian Peninsula (estimated to be between 350 000 and over 3 million, depending on source) and even greater numbers of stray cats, we may be having a ticking bomb scenario.

To avoid this, a true One Health approach is required urgently. One group of collaborators that should not be excluded are volunteer animal rescuers active in the region.

These volunteers, bring much needed passion, dedication and expertise into the direly needed but humane control of animal populations (TNR or Trap-Neuter-Release is a classic example) but not at the expense of animal welfare. I have been privileged to work with some of them.

協力者として外せないのは、この地域で活躍する動物救済のボランティアである。これらのボランティアは、動物福祉を犠牲にすることなく、人道的な動物管理(TNR:Trap-Neuter-Releaseが典型例)を行うために必要な情熱、献身、専門知識をもたらしてくれる。私は、そのような人々と一緒に仕事をする機会に恵まれた。

 

Positively, the tourism inertia in some countries in the region could drive the One Health paradigm needed to avert a monkeypox-like rabies crisis. The time to act is now.

Back to this specific case: was the child bitten and/or scratched? Some people, including health care workers, assume scratches (especially from cats) are harmless -- not requiring post exposure prophylaxis. In one such case, such an assumption precluded a scratch victim from getting post exposure prophylaxis, with an eventual fatal outcome.

With few exceptions, rabies circulation among domestic, stray and wild animals in the Middle East has been considered, since decades, to be underreported. This is also reflected in ProMED's reporting. Since ProMED-mail's inauguration (1994), there have been no rabies reports from Jordan. Lebanon's 1st report has been just published. There were 2 reports from Egypt (both in 2021) and Syria (2013, 2015); one from Iraq (2004; 2 additional reports addressed cases originating in Iraq, identified in the USA in 2008) and just one case, identified in Canada in February 2022, originating/infected in Iran.
一部の例外を除き、中東の家畜、野良猫、野生動物の間での狂犬病の伝播は、数十年来、報告不足であると考えられてきた。これは、ProMEDの報告にも反映されている。ProMED-mailが開始されて以来(1994年)、ヨルダンからの狂犬病報告はない。レバノンからの第1報は、発表されたばかりである。エジプト(いずれも2021年)とシリア(2013年、2015年)から2例の報告があり、イラクから1例(2004年、2008年に米国で確認されたイラクに由来する症例について2例の追加報告)、イランに由来し、同国で感染し、2022年2月にカナダで確認された1例のみである。
