ProMED continues to play the laudable and much needed leading role highlighting how under-reporting blurs out the true incidence of rabies with adverse consequences on its control/elimination. Unreported outbreaks occur elsewhere. Since January 2019, we have, along with collaborators as well as the UK-based Physicians Across Continents (PAC), been monitoring a rabies outbreak in Maiduguri, Nigeria in the heart of the northeastern Nigeria insurgency.

 ProMEDは、狂犬病の過小報告がいかに制御や根絶に悪影響をあたえるかに光を当てるという、賞賛に値する、そして必要とされる役割を担い続けてきた。報告されない(狂犬病の)発生はどこでも起こっている。2019年1月から、我々はイギリスを拠点とする「大陸の医師団(PAC)」のような様々な組織と連携し、ナイジェリア北東部の暴動の最中に起こった狂犬病の監視を行ってきた。

 

Mid-January [2019], a 67-year-old internally displaced person died of rabies a month after being bitten by a dog that bit 15 others. All of them did not receive post-exposure prophylaxis: RIG and anti-rabies vaccines are not available.

 (2019年の)1月中旬に、67歳の国内避難民が、1匹の犬に咬まれたその1ヶ月後に死亡した。この犬はほか15人に咬みついた。その(咬みつかれた)全員が暴露後治療を受けておらず、また使用できる抗狂犬病免疫グロブリンも抗狂犬病ワクチンもなかった。


Another case of human rabies, imported from the Indian subcontinent to the Arab Peninsula, was described in a 2017 report (see reference below). It involved a patient from India who died in Kuwait. Rabies was discovered only after the deceased patient's organs were transplanted, and 2 recipients, in Kuwait and Riyadh, developedclinical rabies. If the permission to harvest her organs had been rejected, that rabies case would have been missed. As the Gulf region is still dependent on migrant workers from these areas where rabies is intensely endemic, it will be prudent for clinicians in the area to have a high level of suspicion and retrieve ante-mortem samples: autopsy is often denied or not carried out.

 もう1つのヒト狂犬病の事例として、インド亜大陸からアラビア半島への移入例が2017年に報告されている(下記引用参照)。これは、インドから来た患者がクウェートで死亡したことに関係する。死亡した患者の臓器がクウェートとリヤドの二人の患者に移植され、その二人が発症して初めて死亡したその患者が狂犬病であったということが分かった。もし、(事前に彼女が狂犬病にかかっていたことが分かっており)彼女(患者)から器官をとる許可が下りていなかったら、この狂犬病の事例も起こっていなかっただろう。湾岸地域のようなところでは、未だに狂犬病が多く発生している地域からの移民の労働力に頼っているため、この地域の臨床家においては、強い疑いの念を持ちながら生前の検体を取り扱う方が賢明だろう。生検はしばしば断られるか、行われていない。


Finally, the site of the bite on the patient's body was not mentioned. A distal location, combined with lower virus concentration, could, among other factors, clarify the long incubation period. Although details are not provided for this case, aggressive wound washing serves to remove rabies-laden saliva from wounds.

 最後に、患者の咬みつかれた部位については記述されていない。末梢の部位、低いウイルス力価、そのほかの要因が組み合わさり、長い潜伏期間となり得る。この事例では詳細が明らかになっていないが、傷口を徹底的に洗うことで、そこから狂犬病(ウイルス)が含まれる唾液を取り除くことはできる。

[The abstract of the mentioned reference follows:

"Rabies is usually transmitted to humans through bites of infected animals; however, it can rarely be transmitted through deceased donor organs or tissues when not suspected. Here, we report a case of rabiestransmission in a child. The child was a 5-year-old girl who was admitted to the pediatric intensive care unit with encephalitis of unexplained cause 3.5 months after she received a kidney transplant from a deceased donor.

 “狂犬病は、通常感染した動物に咬まれることでヒトに伝播される。しかし、まれに(狂犬病と)疑われていない提供者の臓器や組織を通じて感染することがある。ここでは、我々は移植による子供への狂犬病伝播の事例を挙げる。その子供は5歳の少女であり、死亡した提供者から腎臓を移植された3ヶ月半のちに、原因不明の脳炎によって小児用の集中治療室に収容された。

 

 The laboratory and imaging studies did not reveal any explanation for her rapidly declining clinical andneurologic condition, which ended with death 4 days after admission. Death of another recipient from the same donor led to an investigation that revealed rabies as the cause. Both corneas were explanted from other recipients to prevent further death. Polymerase chain reaction sequence analysis of the corneas was consistent with a rabies virus from the same donor's state of residence. Rabies transmission, although rare, should be suspected when a donor comes from or has visited endemic countries. Donors with unclear causes of death should be rejected."

検査機関も画像診断も、彼女の急激な臨床的および神経学的状態の悪化にいかなる明確な説明をつけることができず、収容4日後に彼女は亡くなった。同じ提供者から移植を受けたもう一人の患者も死亡し、検査から死因は狂犬病であったと判明した。さらなる死亡事故を防ぐため、すでに同じ提供者からの角膜を移植されていたほかの患者たちから、角膜が取り出された。その角膜を用いたPCRおよびシークエンスにより、提供者が住んでいた州由来の狂犬病ウイルスの遺伝子が見つかった。狂犬病の伝播はまれではあるが、提供者が(狂犬病の)発生地域出身であるか、そこを訪れたことがある場合は用心した方がいい。提供者が原因不明で亡くなっている場合は(移植を)断るべきだ。”
